Our OHT client required a comprehensive current state review of existing inpatient/outpatient psychiatry services and associated patient journey pathways within the OHT’s catchment area. Leveraging current state review data gathered from diverse stakeholders and applying a population health lens to system planning, recommendations for optimization of psychiatry services and referral pathways were formulated.
Collaborating to Create Integrated Care Pathways
Our OHT client required change advisory and facilitation support for multiple stakeholder engagement sessions to map and validate current and future state care pathways for patient populations living with heart failure and chronic obstructive pulmonary disease. Our OHT client required facilitation support to convene a diverse cross section of partners to discuss and decide how to best activate and implement key elements of their recently ratified strategic plan. Digitalization of Diagnostic Imaging and Scheduling Departments
Our acute care client required an assessment of organizational readiness for a forthcoming transition from a paper-based system for managing medical imaging requisitions to a fully digital solution. YDS leveraged stakeholder insights to analyze change risk, project health, and overall state of organization’s readiness for the digital solution implementation. A robust change strategy was developed and change management training/coaching/advisory services were provided to support successful implementation and proficient adoption of the new digital solution.

Our acute care client required a comprehensive review of emergency department and admissions data to determine the source of an increase in inpatient admissions. We extensively engaged internal/external stakeholders to identify potential partnerships, opportunities, and strategies to safely reduce avoidable emergency department visits and inpatient admissions by better meeting the current and future health needs of the community.

Our acute care client required a comprehensive review of outpatient service volume data. We extensively engaged relevant stakeholders to identify potential partnerships, opportunities, and strategies to safely deliver ambulatory care services in the most ideal setting/location.
MyChart Implementation
Our acute care client and their Family Health Team partner sought to implement a digital solution that enabled patients and caregivers secure access to their electronic medical records (EMRs), aiming to enhance patient engagement, support transparency, and improve care continuity. YDS was engaged to provide change management advisory support throughout the project’s planning and implementation phases.
Ensuring Amalgamation Success
Our not-for-profit homecare client required change management advisory services throughout their acquisition of an adult day services organization. A robust change management strategy was developed using key findings from change management assessments and staff insights.
Virtual Care Implementation
Our not-for-profit homecare client required change management advisory services throughout the implementation of new virtual care technology options.
